Certificate Upon Request

Requirements:

  1. Personal appearance of the person requesting the Certificate Upon Request.
  2. Complete the application form in full. It must be filled out completely.
  3. Document proving your nationality (original and one letter-size copy).
  4. Valid official identification (must contain photograph, full name, and signature), original and one letter-size copy (both sides).
  5. One recent color passport-sized photograph: white background, facing forward, no glasses, and with head uncovered. The photograph must be recent (no more than 30 days old).
  6. A signed statement from the applicant specifying exactly what they wish to have recorded in the Certificate Upon Request, as well as the documents through which they prove the right or fact. Include one letter-size copy. You must write your statement clearly in the application form. If you need more space, you may attach a separate sheet. Important: Only statements of up to 296 characters (including spaces) will be accepted.
  7. Payment of $96.00 USD for consular fees.
