Certificate of Survival (Pensioners' Roll Call / Life Certificate)
This procedure requires an appointment (mandatory).
REQUIREMENTS: All identifications must be current/valid, with full first and last names. Copies must be of both sides and clearly legible.

The applicant must appear in person at the Consulate.

Pensioner ID card or most recent payment stub (original and one copy).

One valid identification (original and one copy):
- Passport
- Consular Registration (Matrícula Consular)
- Voter ID card with photograph (Credencial de Elector)
- Military Service Record (Cartilla del Servicio Militar)
- Texas driver's license or state ID, only if your full first and last names appear on it.

One passport-sized photograph with white background and no glasses.

IMPORTANT – ONLY FOR IMSS PENSIONERS:
- The applicant must inform whether they are a former IMSS employee.
- If they are NOT a former IMSS employee, they must indicate whether they are pensioned under the 1973 Law or the 1997 Law.

In the email, include: your full name, current address, phone number, the procedure you wish to carry out, and confirmation that you already have all the complete requirements.
On the day of your appointment, you must appear without companions at the Consulate to submit all the original documents along with the respective copies. Once received, they will be reviewed and approved, and then your document will be issued to you.
